.: Application Requirements for Foreign Students

Submit the following: (Admitting Officer/Dean)

• Transcript of Records
• Diploma or certificate of Completion for high school graduates
• Letter or Recommendation from the Dean, Principal, Guidance Counselor, Disciplinary Officer from school last attended by applicant.
• Admission Test Result (given by the Guidance and Evaluation Center of USLS)
• Undertaking (download PDF from)
• Birth Certificate
• Payments:
  1. Admission Test- Php : 750.00
  2. Admission Fee - Php: 250.00
  3. Processing of Student Visa - Php 1,500.00
  (This includes Lawyers payment for Notarization of documents but does not include the cost for the processing of student visa and the ICARD or International Card of the applicant.)

FOR STUDENT VISA APPLICATION:

(All documents are to be submitted in 3 copies. (2) Photocopy (1) Original)

1. Five (5) copies of the 1998 Revised Original Personal History Statements (PHS). Duly accompanied and signed by the applicant in English and in National alphabet, accompanied by personal seal, if any, original left and right hand prints on PHS and original photos.

2. Transcript of Records/scholastic records, duly authenticated by the Philippine Embassy or consulate in the applicant's country of origin or legal residence.

3. "SEEN AND NOTED" stamp is not acceptable.

4. A notarized Affidavit of support AND proof of adequate financial support to cover expenses for student's accommodation and subsistence, school dues and other incidental expenses. For scholars, certificate of scholarship from the institution concerned.

5. An authenticated police clearance from the country of origin or legal residence.

6. Original copy of birth Certificate authenticated by the Philippine Embassy or consulate in the applicant's country of origin or legal residence.

7. Medical health Certificate issues by an authorized physician including standard-size chest X-ray needed for quarantine clearance. This can be done here in the accrtedited hospital of the university as soon as the applicant submits the requirements to Admissions Office.

8. Photo copy of Passport pages where name, photo, birthdate and birthplace appear.

9. Certified true copy of the Certificate of eligibility for Admission (CEA) for students of Dentistry and Medicine.

10. National Bureau of Investigation (NBI) clearance as soon as the applicant submits admission requirements to Admissions Office.
