• College Transcript of Records
• (NMAT) National Medical Admission Test
• Birth Certificate
• Recommendation from the Dean where the student obtained his/her Bachelor of Science courses (Recommendation form is provided by school)
• Duly-accomplished Application Form
• 2 x 2 picture - 4 copies
• Medical Certificate from USLS Clinic (including Chest X-ray)
• Psychological Test Result
• Application Fee Receipt

Procedures:

• Submit the following documents to the ASAO Officer:
• College Transcript of Record
• NMAT Score Sheet
• Birth Certificate
• Recommendation from the Dean where the student obtained his/her BS courses major in Science
• Pay Testing Fee of Php. 300.00 and take the Psychological Test from GEC.
• Submit for a medical check up at USLS clinic and secure medical certificate. Present chest X-ray from designated hospital of USLS.
• If qualified, pay application Fee of Php200.00. Fill up Application Form and attach pictures.
• Submit duly accomplished Admission Form.
• Submit to panel interview.
• Secure admission card.
