Admission Steps

Student gets payment form from the Guidance Office and pays the Admission Processing Fee & Psychological Exam Fee at the Business Office or through identified payment centers.

Payment Centers:

  • USLS Cashier
  • MLhuillier – Please fill up Name of student, Student Application Number and purpose of payment

Fees (Non-Refundable):

  • Psychological Exam Fee: ₱300 (College of Medicine)
  • Admission Processing Fee: ₱300

For Online Payments:

Student registers and uploads the receipt of their Admission Processing and Psychological Examination Fee payment through this link: https://forms.gle/dw5CiAAG7jKAEgrg7

GEC sends them the link to the Psychological Examination which should be completed within five (5) days of receipt.

For On-Site Payments:

Student presents the payment form and receipt to the Guidance Office and accomplishes a registration form containing their contact details and email address where the Exam link will be sent.

Schedule of Testing & Releasing of Results:

Cut-off of Payment / Uploading of Receipt & Registration Sending of Links
Monday to Wednesday (12NN) Wednesday Afternoon
Wednesday (PM) to Friday (12NN) Friday Afternoon
Releasing of Results (emailed to their respective college) Three working days after completion of exam

*Note: Student may proceed to the succeeding Admission Steps upon completion of the Psychological Exam.

Student emails the following application requirements to medicine@usls.edu.ph:

  • Application Letter
  • Scanned Copies of their Official Transcript of Records of their Undergraduate Course
  • Curriculum Vitae / Resume with 2x2 Picture
  • Copy of Psychological Test Results (emailed directly to their respective colleges)
  • Recommendation Letter (available in hard copy from their respective College offices or may be downloaded from the USLS Website / Official College Facebook account)
    *Note: Recommendation Letter may be submitted in hardcopy enclosed in a sealed long white envelope to the College of Medicine: medicine@usls.edu.ph
  • NMAT Result – 60% cut-off
  • General Weighted Average Grade Certificate

*Take Note: Entrance Exam Results will be emailed directly to the College of Medicine. Please wait for the confirmation of the schedule for Interview.

Student proceeds for their interview with their Deans.

Student creates an AIMS account through: https://aims.usls.edu.ph/aims/applicants then click APPLY HERE (red button).

Student also uploads the following Admission Requirements to their AIMS Account:

  • Transcript of Records (TOR)
  • Transfer Credentials / Honorable Dismissal
  • College Diploma
  • NSO or PSA Birth Certificate (to request a new copy, please click the link: https://psahelpline.ph)
  • Admission Fee Receipt (₱300)
  • Entrance Exam Receipt
  • Signed Undertaking Form
  • Medical Clearance from the University Clinic – submit the copy of the following to the clinic for clearance:
    • Signed Physical Examination Form (to be accomplished outside USLS) – get downloadable form
    • Chest X-Ray Result (valid within 6 months prior to Admission Application)
    • Philhealth Updated Member's Data Record (MDR) or enrollment form
    • COVID-19 Vaccination Card
    • Medical requirements (to be submitted once admitted to the College of Medicine): HBsAg or anti-HBS, Chest X-ray PA view, Urinalysis, Fecalysis & CBC Results

Additional Authenticated Requirements of Students Coming from Abroad:

  • Grade Interpretation or Equivalence
  • Course Syllabus or Subject Description
  • Recommendation Form in Narratives
  • Foreign Passport and/or Philippine Passport
  • Medical Insurance

After verification of accuracy of documents by ASAO, the Admission Card will be released to the student through email. A hard copy of which may also be generated from the Admissions and Scholarships Administration Office.

Reminder: Submit all requirements in hardcopies to the College of Medicine office.

*Note: Students who encounter problems in the creation of their AIMS account and/or uploading of their documents may visit the Admissions and Scholarships Administration Office for assistance.

Admission Questions & Answers

  • Filled out application form (Get from the Office)
  • NMAT results (60% and above) – taken in the previous two years
  • Application Letter addressed to the Dean (Prof. Radela Yvonne R. Cortes)
  • Official Transcript of Records signed by their school Registrar or Official List of Courses taken and final grades for each course completed up to the time of application with General Weighted Average / Quality Point Index / Cumulative Grade Point Average. (For graduating students, a Certified True Copy of Grades from the Registrar can be submitted for evaluation purposes only. Official Transcript of Records should be given upon enrollment.)
  • Diploma or Certificate of Graduation (For applicants who have already graduated).
  • 2 pieces, 2x2 ID picture (taken within 6 months)
  • Photocopy of live birth certificate from the Philippine Statistics Authority (Certificate of Naturalization, if a naturalized Filipino).
  • Two (2) Certificates of Good Moral Character from the following personnel of the institution where the applicant has graduated or is presently enrolled: Dean, Associate Dean, or any Department Chair.
  • Official receipt of application fee payment
  • Sworn Statement (Get from the office)
  • Handwritten one-page essay (maximum of 100 words) on why you want to be a doctor
  • Psych test result

If you get accepted in the College, you will be asked to submit the following:

  • Photocopy of latest Income Tax Return of parents
  • Certificate of Residency or Voter's ID
  • Active and currently paid Philhealth Receipt and MDR

Take your NMAT in January and February so that you can apply for Academic Year 2026. Otherwise, you are welcome to apply for Academic Year 2027–2028.

The application process is very competitive — only the TOP 50 Applicants will be invited for admission. This is part of the College's thrust to enhance and maximize the transformational learning it provides, aligned to the Mission of the College.
