The Center for LifeSkills is back and is now accepting applicants for Cookery NC II, TWSP scholarship grant under TESDA.

- Form 138 for Recent High School Graduates
- Transcript of Records for Professionals
- Evaluation Transcript for College Level Students
- 7 pcs passport size picture (TESDA specified)
- 5 pcs 1x1 picture
- NSO authenticated Certificate of live birth
- NSO authenticated Marriage contract (married females)
- Medical Certificate/Fit to attend training
- Fill up the Personal Data Form
- Undergo the admissions interview
