Below are the requirements and steps to guide you in your admission to the University.

Eligibility for Application

An applicant must be any one of the following:

  1. A Grade 12 student expected to graduate at the end of Academic Year 2018-2019;
  2. A Grade 12 student who graduated from a school based abroad prior to AY 2018-2019 and who has not taken any college course;
  3. A 4th Year High School student from a school in the Philippines who graduated prior to AY 2018-2019 and who has not taken any college course;
  4. Passer of the Philippine Educational Placement Test (PEPT), Alternative Learning System (ALS) or Non-Formal Education Accreditation and Equivalency (NFEA&E) Program following DepEd regulations and therefore certified eligible for admission to college/tertiary level;

PRE-ADMISSION STEPS

Requirements:
2 pcs 2x2 ID picture with white background
Testing Fee: Php 200.00

  1. Fill out Entrance Exam for Freshmen at Guidance and Evaluation Center (GEC) located at ground floor, Benilde Building
  2. Pay testing fee at the USLS Main Cashier.
  3. Present official receipt at GEC and get exam schedule.
  4. Take the scheduled exam indicated on the permit. No permit, no exam.

1. Online Admission Form

2. Pay Admission Fee of P 200.00 at University Cashier

3. Submit the following at University Clinic:

  • Accomplished Physical Examination Form Record (Examined by your personal physician)
  • Chest X-Ray Result
  • 1pc 2x2 ID Photo with white background

4. Submit and enclosed admission requirements in a long white folder:

  • Original UBAT Result (All Liceo SHS applicants are not required to take the entrance test.)
  • Original Grade 12 Report Card

For ALS Graduate: ALS Report Card and Certificate of Eligibility to college from DepEd

  • Verification and Data Consent Statement Form w/ OAR Number
  • Undertaking Form with 1x1 ID photo
  • 1 Accomplished Recommendation form
  • NSO Birth Certificate (1 original and 1 photocopy)
  • Admission Fee Receipt
  • For Nursing Applicant’s additional requirements:
    • Nursing Aptitude Test Result and HBsAg Result
  • For Dual Citizen’s additional requirements:  
    • Certificate of Eligibility from DepEd and photocopy of both Philippine and Foreign passport bio-pages
  • For International student’s additional requirements:
    • Photocopy of Foreign passport bio-page, Certificate of completion/ Diploma, Course Syllabus with grade interpretation (all documents must be in/ with English Translation)

5. Interview and Evaluation of Requirements

  • MM 11 – College of Engineering and Technology
  • MM 12 – College of Arts and Sciences
  • MM 13 – Yu An Log College of Business and Accountancy
  • MM 14 – College of Education

6. ID Picture-taking at University College Library

7. Issuance of Admission Card at Admission Office

You may start paying the upon enrollment fee at the Cashier after receiving your Admission Card.

ENROLMENT STEPS

1. PAYMENT: (Requirement Admission Card)

After the Issuance of Admission Card, students need to pay the amount required upon enrollment or pay the full tuition at the MAIN CASHIER.

2. SECTIONING, ADVISING AND RESERVATION

After completing STEP 1, please proceed to REGISTRAR'S OFFICE WINDOW 5 or WINDOW 6 for the sectioning, advising and reservation of course

3. PRINT, SIGN and VALIDATE

After completing STEP 2, Kindly print your Enrollment Form in the Satellite ITC area, sign the Enrollment form and leave a validated copy in the area
